2011 - 2012 Athletic Physicals and Registration

Dr. Amerigo N. Ceccarelli, MD FAAP, from Tri-State Pediatrics, will again be conducting the physicals and completing the physician release form.  Physicals will be $10 per athlete.  Cash payment is preferred, but checks for the physicals can be made payable to Dr. Ceccarelli. 

All students must have a physician release form, current for the school year, to be eligible to participate in the St James Athletic Program. 

Registration forms and athlete emergency information forms will also be available for completion at this time.  Bring your student athletes and a separate check for the registration fees, made payable to SJAA.
